If you received the Mail Notice as a nominee for a beneficial owner who holds or held Columbia Mutual Funds, then within fourteen (14) days of your receipt of the Mail Notice, you must either: (a) supply the names and addresses of such beneficial owners to the Settlement Administrator; or (b) request additional copies of the Mail Notice from the Settlement Administrator and within fourteen (14) days of receipt of the copies of the Mail Notice from the Settlement Administrator send the Mail Notice to all beneficial owners. Nominee purchasers who elect to send the Mail Notice to their beneficial owners shall provide a statement to the Settlement Administrator confirming that the mailing was made as directed.

2. Call (877) 690-7105 to request Mail Notices or to request a secured FTP site to deliver the names and address of beneficial owners who should receive a Mail Notice.

If you chose option (b) above, upon receipt by the Settlement Administrator of proper documentation, you will be reimbursed for the reasonable expenses of sending the Mail Notice to beneficial owners.
